Telehealth consultations

Dr Monique is happy to complete your consultation remotely either via Zoom, or over the phone. When you book a telehealth consult, you will be sent details of how to connect. Please note, if you require a physical examination Dr Monique may request you to schedule an in-person appointment.

Parking and Getting to your appointment

Q Central parking: Limited free parking is available in the Q Central Building (2.3m height restriction). If there is no visitor parking available please proceed to the boom gate at the end of the undercover parking and dial “101”. If you have an appointment scheduled with Dr Monique you will be let through. Please park in one of the designated “Genea” parking spots.

Free parking may also be available in the surrounding streets.

Alternatively, you may be able to park at the Circa Retail Shopping Centre which is a 300m walk to the Q Central Building.

Paid parking is available across the road at Norwest Private Hospital.

If you have trouble finding us on the day of your appointment, or are running late, please phone us 02 9052 6482.

Lifts are located on ground floor next to the café. Take the lift to Level 1. Turn right out of the lifts, and right again. Dr Monique’s consultation rooms are located in Suite 111 on your left.

Appointment re-scheduling or cancellations

Please phone our clinic if you need to reschedule or cancel your appointment. We appreciate at least 24 hours notice so that we can offer your appointment time to another patient. Appointments cancelled within 24 hours may attract a cancellation fee. If you wish to reschedule your appointment within 24 hours, pre-payment will be requested prior to re-booking your appointment.

Dr Monique works with Gabby as her personal assistant. Gabby is passionate about making your clinic visit as comfortable and streamlined as possible so please call her 02 9052 6482 with any questions you have or to make an appointment to see Dr Monique.

If you proceed to having fertility treatment this can be arranged through Genea Bella Vista. Dr Monique is proud to work with Genea as it has consistently yielded some of the highest success rates from IVF cycles across Australia. Your treatment will be personally managed by Dr Monique, and you will receive support from the experienced nursing, scientific, counselling, genetic and administrative teams.

What to bring to your first appointment

Gabby will ask you for the following information when scheduling your appointment:

• A current referral from your GP (valid for 12 months) or another specialist (valid for 3 months) – this will allow you to claim the Medicare rebate to see Dr Monique as a specialist. If you are presenting for fertility treatment as a couple, both patient names need to be listed on your referral.

• Details of your usual GP (if different from your referring doctor),

• Medicare, DVA and/or Pension card,

• Private Health Insurance details, if applicable,

• A copy of any test results: blood tests and imaging reports relevant to your presentation,

• A copy of any correspondence from other specialists involved in your care.

You will be asked to complete a brief medical questionnaire which allows Dr Monique to understand a bit of your background before meeting you.

If you are presenting for fertility treatment as a couple, you are both encouraged to attend. Please feel free to bring a support person and a list of questions you may wish to ask so you don’t forget on the day.

Payment details

We are a private clinic and charge private-practices fees.

Full payment is required on the day of your appointment. You will receive a receipt so that you can claim the rebate from Medicare.

Full upfront payment in advance is required for all telehealth consultations. You may pay at the time of booking, or we will contact you a week before your appointment to arrange.

Private health insurance does not cover consultation costs.

Our clinic does not accept payment in cash. You may pay using EFTPOS – most major cards including VISA and Mastercard are accepted. AMEX/Diners will attract a fee of 2.48%.

For referrers

Dr Monique is happy to receive calls from referring GPs or specialists to discuss a particular patient.

Referrals and any relevant investigation results can be sent via email or fax (02 9052 6483). Please use your own, or the attached template.

If you wish your patient to be seen urgently, please phone our clinic. Dr Monique will be able to triage your patient to make the most appropriate arrangements.
